Spring 2011 Waiver Calendar and Process 

 

Dates:

. February 1 - February 21: window open to submit waivers to counselors

. February 28- Counselors will be notified about PE waivers that do not meet

minimum requirements

. May 23 - May 27: Spring waiver verification due to counselor

. June 3rd Submit waivers to registrar

 

Process:

If meeting requirement through the YMCA

1. Counselor will sign PE Waiver

2. Student will submit the waiver to the YMCA for approval

3. Student will attend the YMCA orientation on Wednesday, February 16 at 5pm

4. YMCA staff will collect and sign the waiver once the student has
completed their

hours.

5. Staff from the YMCA will sign and submit the waiver to the VP for final
approval

by 6/3/2011.

 

If meeting requirement through another sports activity that is part of an
organized

program

 

1. Student will fill out their portion of the waiver and submit all the
signatures and

required documentation by February 17.

a. Required documentation due before February 17th: signature of coach and

letter on official letterhead from the participating organization verifying

enrollment in program.

b. Required documentation due by May 27th: signed waiver. If the signature

on the carbon copy differs from the first signature then an additional
letter

on official letter head is required.

 

2. Counselors will provide student with carbon copy of the waiver

a. Counselors will submit original waiver and brochure to VP by February

21st

b. VP will notify counselors by February 28th of students who do not meet

minimum requirements

c. Vice Principal will sign and submit waiver when all of the criteria has

been met
