Hello BHS Community, 
The City of Berkeley Public Health Department and BUSD are continuing to offer free COVID-19 vaccinations for everyone age 12 and older at vaccination pop-up clinics at Berkeley High School and Longfellow Middle School.  If your student already received their first vaccine, they will be notified of their return date for their second vaccine. 
 
First vaccines are also available at all clinics on a walkup basis. There is no need to register.
 
Clinic Dates and Times
 Across from Berkeley High School at Civic Center Park
June 2-4
10:30 a.m. – 6:30 p.m.
 
Longfellow Middle School
June 8, June 11, June 14-16
11 a.m. – 6:30 p.m.
 
Parent/Guardian Consent Required
Students must have permission from a parent or guardian to be vaccinated. There are several ways to demonstrate parent/guardian consent:
Handwritten consent
A call to a parent/guardian from the vaccine site
Parent/guardian accompanies student

Hola Familias de BHS,

El Departamento de Salud Pública del Gobierno de la Ciudad de Berkeley y BUSD continúan ofreciendo vacunas gratuitas COVID-19 para todas las personas de 12 años de edad y mayores en las clínicas de vacunación emergentes (pop-up) en la Escuela Preparatoria Berkeley High y en la Escuela Secundaria Longfellow. Si su estudiante ya recibió su primera vacuna, se le notificará la fecha que tiene que regresar para su segunda vacuna.
 
Las primeras vacunas también están disponibles en todas las clínicas sin cita previa. No es necesario registrarse.

Se Requiere el Consentimiento del Padre/Tutor Legal
Los estudiantes deben tener un permiso de su padre o tutor legar para ser vacunados. Existen varias formas de demostrar el consentimiento del padre/tutor legal:
Consentimiento por escrito
Una llamada telefónica del padre/tutor legal del personal en el establecimiento de vacunación
El padre/tutor legal acompaña al estudiante
