FALL TEAM TRYOUTS: Monday, August 13 through Friday, August 17, 2012
(Football on Aug. 6) 

WHAT DO I NEED TO DO TO TRY OUT FOR A TEAM?

*	Attend Team Tryouts ALL 5 DAYS starting August 13 (Except Football
which starts August 6).
*	Bring appropriate gear and water.
*	Complete an Athletic Packet including a Medical Examination form.
The form must be signed and stamped by a doctor who verifies that you are
healthy to tryout and compete in high school sports.  Physical forms are
good for one year only.
*	Destroy old athletic packets.   
*	Complete new, revised packet found at:
www.berkeleyhighathletics.org/athletic-packet
*	Remember to include all signatures for doctor, student athlete, and
parent/guardian on emergency forms, medical examination, and concussion
forms.
*	Bring athletic packet to first day of tryouts.   No Athletic Packet=
No Tryout.
*	Questions or Concerns?  Contact the head coach.
