Student Athletes

 

Do you still need a medical examination for sports team try-outs?  Remember
that the exams are good for one year only.  If so, get a thorough physical
at Children's Hospital and complete the required athletic packet.  

What:  Physical examinations for Berkeley High sports teams

When: August 13, 2011 from 9:00-11:00 AM

Where: Sports Medicine Center for Young Athletes, Children's Hospital, 744
52nd Street, Oakland- Ortho Clinic
 
How: Pay only $30 per athlete.  Call to sign up for an exam at 510-428-3558
Option 3

Call today for your physical.  Complete the mandatory athletic packet before
tryouts on August 15 for fall sports, November 7 for winter sports, and
February 6 for spring sports. 

Athletic packets are available outside the AD office or on-line at
www.berkeleyhighathletics.org <http://www.berkeleyhighathletics.org/> 

Children's Hospital requires a parent signature on a separate form.  Go to
www.berkeleyhighathletics.org/physicals  to print the Children's Hospital
form before August 13
