[BHS etree] ADMIN: H1N1 Clinic, Last minute instructions for students and volunteers

1. On Tuesday and Wednesday and Wednesday of this week (December 8 and 9),
the city of Berkeley, working in conjunction with the BHS Health Clinic,
will be holding a H1N1 Flu clinic at Berkeley High.  In order to e
vaccinated, all students must bring a fully filled out release form.  This
form along with H1N1 vaccination information was mailed home before
Thanksgiving.  If you have not received yours, or have lost it, please
either download a new one at www.ci.berkeley.ca.us/publichealth or pick one
up at the Front Desk.

2.  There are no nasal vaccinations available.  All vaccinations will be by
injection.

3.  Adult volunteers will pick up students with signed in waivers in their
History classes.  Other volunteers will check paperwork and help manage
traffic.

4. If you have volunteered to help for either a morning (8 to 11:30) or
afternoon (noon to 3:30) shift, please come by the Front Desk in the BHS
Admin building to first sign in and get an official H1N1 Volunteer badge and
then head to the foyer of the Community Theater.  Lisa Sterner, director of
the BHS Health, will meet you there for training and assignments. Please be
prompt as the clinic will start as soon as the morning or lunch bell rings.
