BHS Health Center H-105 FIRST floor H-BUILDING

The Berkeley High School Health Center is a safe, nurturing, dynamic,
multicultural teen health center. Teens are encouraged to develop the
skills they need to take ownership and responsibility for their own health
and wellness as well as that of the larger community. The Health Center
provides access to culturally appropriate care through a combination of
direct services, partnerships, and referrals to community resources.

Health Center Hours:

Monday-Friday 8:30 AM - 4:00 PM

Closed Daily: 12:00 PM - 1:00 PM

FIRST AID Treatment for minor injuries and illnesses, immunizations, and

Parent/Caregiver must complete a consent form for first aid services.

CONFIDENTIAL MEDICAL SERVICES Screening and treatment of STIs, birth
control and condoms, emergency contraception, health education, HIV
prevention and testing.

No Parent/Caregiver consent needed for confidential services.

MENTAL HEALTH Individual short-term counseling, crisis assessment,
emotional support and stress management, consultation for parents and
school staff, and referrals for ongoing MH services and community
resources.

Global Humanitarian Day

This is a message from your chief of service, Gursimar Kaur, to remind you
that this Friday, August 19th is World Humanitarian Day.

This is an international day to remind us of the sacrifices made by Human
Aid workers and volunteers. Amidst life-threatening conflicts across the
globe, we’ve seen Human aid volunteers rise to support the people in need
throughout history despite the dangerous conditions of the work. August
19th is a way to commemorate the lives of humanitarian aid workers lost
each year as well as be appreciative of their hard work towards the good of

Fall 2022 • PE WAIVERS ***updated 8.17 with new PE WAIVER FORM Link***

This waiver form applies only to students participating in an outside
sports program that fulfills the following requirements below. Any student,
participating in a state or nationally sponsored competitive sports
activity, including participation in a professional dance company, can
request a waiver of the Physical Education requirement from the Berkeley
High School Administration.

Waiver Process

1. Student provides Student ID number, grade level and SLC to the coach.

2. Your coach will print and complete all parts of the waiver.

3. Your coach writes a letter on company letterhead

Letter Requirements:

       a. MUST be on letterhead

       b. Your name and the date

       c. Your coach’s contact information

       d.  Assurance that you are meeting state physical standards and
competencies through the activity         e. The specific number of hours
you will be doing the activity - minimum of 60

        f. The coach who writes and signs the letter MUST also sign the
waiver in the beginning of the semester & at the end. Remember all coach
signatures must be by the same coach.



4. Must provide proof of registration and/or billing statement from the
organization where you plan to complete your PE credits requirement. The
registration must be current to Fall 2022 and must have the student’s name
showing he/she is registered.



5. Your Coach must email the PE Waiver Form, letter, and proof of
registration

to pewaivers at berkeley.net no later than Friday, September 9, 2022. It will
NOT be accepted if anyone else sends it. The coach and the student will
receive a confirmation receipt that all documents have been received and
are being reviewed.



Once you have completed your hours, have the same coach sign and email the
original copy of the waiver with the additional signature

  Remember, your activity must:

Be a structured athletic program, or a competitive (state-sponsored) team

60 hours for the semester.





* The Berkeley YMCA is the only gym with a BHS PE waiver program.

ENTERING CAMPUS in 2022 For 2022-23, all students will enter campus through
the M-Gate at Milvia & Bancroft. In the morning, the MLK Gate at MLK &
Bancroft will be open until shortly after the start of 1st period. The A
Gate is closed due to construction. During the class period, you should
enter through the main office at Allston & Milvia and prepare to show your
student ID card upon entry.

FREE BREAKFAST & LUNCH FOR ALL STUDENTS in 2022-23 All students will be
able to receive FREE breakfast and FREE lunch throughout the entire school
year. Meals will be pre-packed, heated and ready to go at the pickup
window. You will pick up the meals from the little window on the outside of
the cafeteria near the main entrance to the D-Building.

EZ MEAL APP Even though a lunch application is not required for free school
meals, your family should consider completing an online EZ MEAL APP
<http://secure.ezmealapp.com/> to take advantage of discounts and other
benefits.

Come to the wrestling room (next to the M Gym) from 4 to 6 on Wednesdays
this month to learn what it's about and meet some of the team. To show up,
you only need to dress in athletic clothes. If you have any questions, you

Notification of Non-Discrimination Policy

The Berkeley Unified School District is committed to providing a
hostile-free working and learning environment. The District prohibits
discrimination, harassment, bullying, intimidation, and retaliation, based
on actual or perceived race, ethnicity, sex, sexual orientation, gender,
gender identity/expression, pregnancy, childbirth, breastfeeding/lactation
status (related medical conditions), religion, color, national origin
(including language restrictions and possession of a driver’s license
(issued under Vehicle Code), ancestry, immigration status, physical or

HIV/AIDS, hepatitis, epilepsy, seizure disorder, diabetes, multiple
sclerosis, and heart attack), medical condition (cancer-related and genetic
characteristics), military and veteran status, marital status, registered

belief or affiliation (unless union related), a person’s association with a
person or group with one or more of these actual or perceived
characteristics, or any other basis protected by federal, state or local
law, ordinance, or regulation in any program or activity it conducts or to
which it provides significant assistance.

Discrimination is different treatment based on a protected characteristic
in the context of an educational program, work or activity without a
legitimate, nondiscriminatory reason that interferes with or limits the
individual’s ability to participate in or benefit from the services,
activities, or privileges provided by the District or includes an adverse
employment action.

Hostile environment harassment occurs when a target is subjected to
unwelcome conduct based on a protected characteristic, which is both
subjectively offensive to the target and would be offensive to a reasonable
person of similar circumstances/characteristics and is sufficiently so
severe, and so pervasive to interfere with or limit an individual’s ability
to effectively work or to participate in or benefit from the services,
activities, or opportunities offered by the District. Harassment may take
many forms, including but not limited to, verbal remarks and name-calling,
graphic and written statements, or any conduct that may be threatening or
humiliating. Harassment based on any of the above protected categories is a
form of unlawful discrimination and will not be tolerated by the District
and can result in disciplinary action against the offending student or
employee.

Upon witnessing an act of discrimination, harassment, bullying,
intimidation, and retaliation, based on actual or perceived protected
characteristics, school personnel are required to take immediate steps to
intervene when it is safe to do so. Once a school/office has expressed
notice or reason to know of such conduct, whether carried out by employees,
students, or third parties, it should take immediate and appropriate steps
to investigate, determine what occurred, take prompt and effective steps
reasonably calculated to end the conduct, eliminate a hostile environment,
if one has been created and prevent the conduct from occurring again. These
steps should be taken regardless of whether an individual makes a complaint
or asks the school/office to take action. Complaints are protected from
retaliation. The District prohibits retaliation against any person who
files a complaint or an appeal, reports instances of noncompliance,
discrimination, harassment, bullying, and intimidation, or who participates
in the complaint-filing or investigation process. This nondiscriminatory
policy applies to all acts related to school activity or school attendance
within any school/office under the jurisdiction of the Berkeley Unified
School District.
