[BHS Etree] ADMIN: At-Home Covid Tests + Covid Vaccine Clinics

Hello BHS Community,

This week all students and staff received a box of iHealth at-home rapid
antigen tests. There are two tests in the box. *For now, please hold on to
these tests.* The California Department of Public Health has asked that
staff and students test on *Friday, December 31* and again on *Sunday,
January 2*. This is an important risk mitigation strategy for our return to
school and work on January 3.

*Please disregard instructions and the QR code on the box that ask you to
download the iHealth app.* We are not using this app for test results.

We are waiting for Primary Health to provide final information about the
process of uploading test results. Once that happens, we will provide
additional details about this process.

We are sharing some opportunities for students to receive the COVID-19
vaccine over the next week, as well as for students ages 16-18 and adult
family members to receive the booster. Children younger than 16 have not
been approved to receive the booster at this time. *Note: All family
members ages 5 and older may receive vaccines at these clinics.  *

*Parental consent:* At all clinics, students under 18 need parent or
guardian permission via signed consent form (download the consent form here
<https://www.cityofberkeley.info/uploadedFiles/Health_Human_Services/Public_Health/covid19/CH%20Minor%20Consent%20Form.pdf>),
in-person consent at the vaccination clinic, or via a phone call. Patients
ages 5-15 must be accompanied by a responsible adult before, during, and
after their appointment.

*Sylvia Mendez Free Vaccine Clinic, Saturday, December 18*

In Partnership with Carbon Health the City of Berkeley is offering a free
vaccine clinic at Sylvia Mendez Elementary School located at 2840 Ellsworth
Street this Saturday December 18th from 9:00-3:30. First and second dose
Pfizer vaccines are available for everyone over the age of 5. Pfizer
boosters are available for those over the age of 16.

vaccine clinics at Berkeley Adult School on Monday, December 20 and
Wednesday, December 22. First and second dose Pfizer vaccines are available
for everyone over the age of 5. Pfizer boosters are available for those
over the age of 16.
