FINAL REMINDER NOTICE



YOUR COACHES FINAL SIGNATURE ON

WAIVERS ARE DUE FRIDAY, DEC 10, 2021.



Please have your coach sign and submit, by their email, the Fall 2021 PE
Waiver form back to pewaivers at berkeley.net with their final signature no
later than Dec 10, 2021.

The email must come from the coach.



*Spring 2022 Semester PE Waivers will be available on December 13, 2021,
with waivers due Jan 14, 2022.
