Dear BHS Family,

On the evening of July 8th, you should have received an email asking you to
fill out a Fall 2020 Instructional Program Choice Form for each of the
students in your household.

The form asks you to indicate your preference for either Distance Learning
or a “Hybrid” On and Off Campus Program when school begins on August 17th.

If you didn’t receive the email, here are the links to the forms in English
and Spanish:

Please submit just one form for each student enrolled in Berkeley Unified
School District. The deadline to submit this form for each of your students
is July 16.

Please note, if you did not receive the email from the District, please
check your inbox and spam folder for an email with the subject line: “Fall
Instructional Program.”

If you don’t find this email, please send an email to: myemail at berkeley.net

Please provide the following information in the email:

   - Your child’s name
   - Grade
   - School
   - Your email contact information
   - Your cell phone number

If you have questions or comments, please send them separately to

Wednesday, July 15 at 7:00 pm, the Superintendent will provide an update on
Fall 2020 at a special School Board meeting. Here is information about how
