Are due back in the counseling office no later than

Friday, January 31, 2020.



Please make sure to attach the letter on letterhead from the coach and
proof of payment.

Waiver Available for Pick up 01/13/2020

Step 1 of the Waiver Due 01/31/2020

Step 2 Completed(yellow) Waiver Due 05/22/2020



Summer Waiver 2020

Waiver Available for Pick up 05/26/2020

Step 1 of the Waiver Due 06/11/2020

Step 2 Completed(yellow) Waiver Due 09/11/2020
