*When students attend registration next week*, *they should bring with them
the following:*

· Completed Emergency Card or Have It Filled Out Online

BHS requires that a new Emergency Card be filled out every year.

10th, 11th and 12th grade students can complete this task in *one*
of the following ways:

1. *Preferred*: Have their parents/guardians activate/access their
*Illuminate
Home Connect* account (parent portal) today! During registration week,
parents/guardians can complete the online emergency card form that will
post to active BHS parent Illuminate accounts. More here.
<https://www.berkeleyschools.net/2018/08/complete-your-students-emergency-card-online/>

2. Use this link
<https://drive.google.com/file/d/0BxP1J3Nz2IFnV09TUmI4SkZweTg/view> to
print out the emergency card and turn in at orientation. Download the
form, print, and fill it out by hand or fill it out on a computer using
Adobe Acrobat, then print.

3. Pick up an Emergency Card at orientation to complete at home and
return to the school office by Tuesday, September 4th.

*Note for 9th grade**: *Incoming Freshman have already provided the
emergency card information and do not need to turn in another one unless
parent/guardian contact and emergency information has changed.

· Signed Health Center Release Form
<https://drive.google.com/file/d/0BxP1J3Nz2IFnQXBrbk1WY2V6MDQ/view?pref=2&pli=1>:
All incoming freshmen should have completed this form during the BUSD
registration process. For 10th, 11th and 12th graders, please submit a
completed form if you are new to BHS, you have *not* completed one in the
past and would like your student to have access to Health Center services,
*or* if there has been a change in health history (e.g. recent diagnosis of
diabetes, seizure disorder etc.

· A lock for their locker (we suggest combination locks)

· Money for ASB (Associated Student Body) Leadership sticker and Jacket
Pride gear. More information can be found at http://berkeleyschools.net/
leadership

· Their personal cell phone number and the password to their school email
to sign up for emergency text message notifications. As part of our school
safety plan, we will be implementing an emergency text message notification
system for students to receive critical information in the event of a
schoolwide emergency. We strongly recommend that all students opt-in to
this service.

<https://docs.google.com/document/d/1DNg6PTu4yT-jrckEiwWGgakg89ZcKi8AaD-ZHjERhzw/edit?usp=sharing>
for updates on our student database system, Illuminate. *Illuminate Home
Connect* gives students and parents/guardians access to assignments,
grades, attendance data, and more. We encourage all parents/guardians to
access the parent portal. Information on how to access it can be found here
<https://www.berkeleyschools.net/2018/08/complete-your-students-emergency-card-online/>.

important messages that you won’t want to miss.

*Athletic Programs* -- Information from our *Athletic Department*, including
the online process for team registration and a link to fall tryouts can be
found at bhs.berkeleyschools.net/athletics/ Many fall sports have already
begun tryouts this week. please review this information ASAP.

*Lunch Program Information* -- We are asking every family to complete an
application for free/reduced price school lunch. Even if your student does
not intend to take advantage of the lunch program, every qualified application
is counted towards bringing significant federal (Title I) and state (LCFF/LCAP)
funding to our schools!

Please visit https://secure.ezmealapp.com/Default.aspx to access the secure
online meal application. *If you qualify* for Free/Reduced Meals the
benefits apply to testing, graduation costs, and other school activities.
Once a student applies and qualifies, the information is automatically
linked to their existing student ID number.

If your student does not qualify for free/reduced price lunch, and you want
an option that will make it convenient for your student to purchase meals
in the BHS Food Court, please visit https://www.ezschoolpay.com/Login.aspx
to pay by debit card.
