<https://drive.google.com/file/d/0BxP1J3Nz2IFnV09TUmI4SkZweTg/view?usp=sharing>:
Incoming Freshman have already provided this and do not need to turn in
another one unless information has changed. Upperclassman can pick up
an Emergency
Card <https://drive.google.com/file/d/0BxP1J3Nz2IFnV09TUmI4SkZweTg/view> at
orientation to complete at home or use the link provided to print out the
form and turn in at orientation. The form can be filled out on a computer
using Adobe Acrobat, after downloading, but must be printed and turned in.
BHS requires that a new Emergency Card be filled out every year.

<https://drive.google.com/file/d/0BxP1J3Nz2IFnQXBrbk1WY2V6MDQ/view?pref=2&pli=1>:
All incoming freshmen should have completed this form during the BUSD
registration process. For 10th, 11th and 12th graders, please submit a
completed form if you are new to BHS, you have *not* completed one in the
past and would like to start receiving Health Center services, *or* if
there has been a change in health history (e.g. recent diagnosis of
diabetes, seizure disorder etc.)

• A lock for their locker (we suggest combination locks)

*For Students Unable to Attend Registration*

On the first day of school (Tuesday, August 29), please check in at the
counseling office in the Admin building, starting at 8:00, where students
can get a printed copy of their schedule. In the Student Daily Bulletin and
during the morning announcements during the first week of school we will
announce Textbook Room hours and share info about photo ID make-ups.

*Students should be checking their student email* for an update on our new
student database system, Illuminate, which has replaced Powerschool.
Information for parents on how to access the parent portal will follow soon
after.

*New this year** --* Information from our *Athletic Department*, including
the online process for registration and a link to fall tryouts can be found
at bhs.berkeleyschools.net/athletics/

*Lunch Program Information* -- We are asking every family to complete an
application for free/reduced price school lunch. Even if your student does
not intend to take advantage of the lunch program, every qualified
application is counted towards bringing significant federal (Title I) and
state (LCFF/LCAP) funding to our schools! Please visit
https://secure.ezmealapp.com/Default.aspx to access the secure online meal
application.

If your student does not qualify for free/reduced price lunch, and you want
an option that will make it convenient for your student to purchase meals
in the BHS Food Court, there is a separate application for a student meal
debit account for charging meals at https://www.ezschoolpay.com/Login.aspx.
