P.E. WAIVER INFORMATION:

Completed Summer 2016 Waivers (for returning students only):

Please turn in your completed Summer PE Waiver (yellow copy) to the Attendance Office beginning today and no later than September 16, 2016 by 4:00 pm. The waiver must be signed by the same coach who wrote your letter early in the fall. No exceptions to this deadline will be granted.

Fall 2016 Waivers

The Attendance Office will begin distributing Fall PE Waiver forms on September 19, 2016. The first portion of the waiver process is due back to the attendance office no later than October 7, 2016. No exceptions will be made for missed deadlines. The Fall waiver will include a cover page with all the information you will need to know. Please make sure you read it.

IMPORTANT INFORMATION:

1. Students who choose the YMCA to receive 5 PE credits do not have to worry about deadlines or picking up a PE Waiver. The YMCA handles and monitors students' credits on their end. Upon completion of their mandated hours, they YMCA turns in a final list of students who successfully completed the required hours (60) to the attendance office for credit. See attached flyer.

2. If your student is participating in a BHS Athletic Team (ie: LAX, Basketball, Football, Soccer, Baseball, etc.) they will receive automatic credit for their participation on the team. If you are wondering about credits for a BHS Athletic Team, please contact the office of the Athletic Director at athleticdirector at berkeley.net<mailto:athleticdirector at berkeley.net>

3. Parents of Juniors and Seniors: Please make sure to review your child's PE credits! They need 20 credits (or four semesters) to graduate. You can find those credits in your child's transcript. Please contact your child's counselor to make a plan if your child still needs to work on PE Credits. If you believe your child was not awarded PE Waiver credits, please email us with the exact semester that s/he is missing so we can look into it.

4. New This Year: Students completing an individual PE waiver must provide proof of registration and/or billing statement from the organization where s/he plans to compete their PE credit requirement. The registration and/or billing statement must be current to the fall of 2016 and must have the students' full name on the document provided. Organizations that do not have registration forms or who do not charge can contact the attendance office at pewaivers at berkeley.net<mailto:pewaivers at berkeley.net> for advice about how to create a registration form.

5. PE waivers are only available in hard copy.
