*Summer PE Waiver* pewaivers at berkeley.net

· Hand out individual summer waivers: May 31st

· Return first part of the individual summer waivers: June 15th

· Summer waivers due in the Fall: September 16th

· Group Waivers: This is for students who are signed up in organizations
like the YMCA. Group waivers are handled by the coaches/organizations who
are responsible for meeting our deadlines. In order for an organization to
qualify to do a group waiver they need to have at least 20 BHS students
signed up.

· Individual waivers: This is a 2 part process. a) Once students pick up
an individual waiver they have a couple weeks to have their coach write
them a letter stating that they are enrolled in their program, and that
they will meet the Ca. Ed code requirement of a minimum of 60 hours of
physical activity. They coaches are also required to sign the first portion
of the waiver form. b) Once students return the letter and signed waiver by
the deadline (in this case June 15th) we give them a yellow form (carbon
copy of the waiver). That form is then signed upon completion of the
required hours and returned to us by the set deadline (Sept. 16th).

· FYI, if students wish to sign up with the YMCA they must have a
membership.
