[BHS Etree] SPORTS: URGENT Information on Athletic Packets

This is the week for clearing Athletic Packets for Spring Sports (February 3-5, at lunchtime and 3:30-4:30 pm in the Athletic Director's office).

It has come to our attention that some packets at the Front Desk were missing the Physician form to be signed, dated and stamped by a Physician/Doctor. This is a VERY IMPORTANT PART. You will not be allowed to clear a packet without this completed form.

Please check your packet and confirm that you have 4 pages to turn in (emergency form, signature/initial page, PHYSICIAN'S FORM, and a personal health history). Each page has places for signatures; please be sure they are signed.

If you picked up a form from the Athletic Director's office, or downloaded the online form, you should be okay but you should double check to make sure that you have all 4 pages.
