Dear Parents,

There have been some problems with the links to the HEALTH CENTER 
CONSENT FORMS on the BHS Registration website.

*To access the CORRECT and COMPLETE Health Center Consent Form packet, 
please follow the links provided in this message.* All information that 
you will need about Health Center services is included in the packet 
provided.

Go to http://bhs.berkeleyschools.net/2014-15-berkeley-high-registration 
and scroll down to REQUIRED FORMS.

If you are a *FRESHMAN* parent, please send all signed forms with your 
student to his/her orientation tomorrow, August 21^st .

If you are a parent of a *SOPHOMORE, JUNIOR, or SENIOR*, please review 
the Health Center Consent packet provided in this link to ensure that 
you have completed and submitted all necessary forms. If not, please 
send a fully completed packet with your student on the first day of 
school and ask him/her to drop it off at the Health Center.

***An incomplete Health Center Consent Packet could result in your 
student being*

*excluded from receiving certain Health Center Services***
