How to have midyear reports sent (if required):

All Common Application midyear reports will be (or has been) sent automatically.  

If your college requires the midyear report and you are not submitting the application through the Common Application, you need to download the midyear report from your college's website and fill in the required student information.  Submit this form to the Registrar's office in D173 as soon as possible.  You should attach a stamped (45 cents) business size envelope addressed to each school.

How to send transcripts only:

Turn in the following to the Registrar's office in D173:
A signed transcript request (available in the Registrar's office) for each college.  If you are under 18 your parent/guardian needs to sign it.
Clip each transcript request form to a business size envelope addressed to the college with Berkeley High School as the return address.  Put one stamp (45 cents) on the envelope.  Clip together and turn them in to the Registrar's office.
