RECOMMENDATIONS AND TRANSCRIPTS SUBMITTED ONLINE THIS YEAR

 

For the first time this year, Berkeley High is submitting counselor letters
of recommendation (Secondary School Report) and transcripts on line for the
Common Application. It is important that you fill out the School Form
information on the Common Application as soon as possible, but no later than
October 7 for Early Decision/Early Action and November 3 for Regular
Decision. Only students who adhere to these deadlines will be guaranteed an
on-time submission of the counselor letter of recommendation.

 

IF YOU ARE NOT SUBMITTING THE COMMON APPLICATION ONLINE, FOLLOW THESE
DIRECTIONS FOR SENDING TRANSCRIPT, COUNSELOR REC., MIDYEAR REPORT VIA MAIL

 

Create packets as follows for each college you are applying to:

*         Fill out a blue transcript request form for each college. You can
get this form in the Registrar's office, D173.  If you are under 18, you
parent or guardian must sign it.

*         The Secondary School Report Form, also known as Counselor
Recommendation or Teacher Evaluation form, can be found by clicking on the
Download Forms link on the Common Application website (or on the college's
website).  Complete the student information before printing out the number
of school reports needed, one for each college.  You also need to sign the
form.  We highly recommend that you waive your right to see the
recommendation. If you do not, colleges may wonder if you are trying to hide
something.

*         Address a large envelope (9x12 inches) to each college with
Berkeley High School as return address.  On each envelope put stamps that
total $1.22 (for the large envelope 3oz. rate.)

*         Clip the transcript request and the Secondary School Report form
for each college to the outside of each envelope. 

*         Clip all the separate packets together and put the College
Application Checklist on top of them all.

*         Turn in these packets to the College Career Center on October 7
for Early Decision/Early Action and on November 3 for Regular or Rolling
Admission.

 

How to send a transcript only:

 

Turn in the following to the Registrar in D173 (not the College Career
Center).

*         A signed transcript request (available in the Registrar's office)
for each college. If you are under 18 your parent/guardian needs to sign it.

*         Clip each transcript request form to a business size envelope
(#10) addressed to the college with Berkeley High School as the return
address.  Put one stamp (44 cents) on the envelope.  Clip together and turn
them in to the Registrar.

 


How to have mid-year reports sent if required:


 

*         You can download this form from the common App website (see
"download forms" option) or from the college's website.  Fill in the
required student information before printing the number of forms required.

*         Midyear Report forms are due to the Registrar in D173 (not the
College Career Center) by January 10.  You should attach it to a stamped (44
cents) business-size envelope addressed to each school.
