[BHS etree] SPORTS: CORRECTION to Sports Physical Announcement

The Health Center is offering Sports Physical exams ONLY for those
students who don't have health insurance or any other way to pay for a
Sports Physical exam. Recognizing that Sports Physicals are one of the few
times students see their regular doctors, we strongly encourage students
who have health insurance to see their regular doctors for this exam.
Health Center staff wants to encourage all students to maintain ongoing
care with your primary doctor.

If you don't have a regular doctor OR a means to pay for a Sports
Physical, then you can receive your exam at the Health Center. Please drop
by the Health Center or call 644.6965 to set up an appointment. For your
Sports Physical, please note that you must:

1. Pick up a BHS Athletic Packet from the Athletic Director's office
located in Donahue Gym.
2. Come to the Health Center with an Annual Physical Examination form (in
your athletic packet) completed and signed by a parent/guardian, in order
to schedule the appointment. We will not perform the sports physical
without the Annual Physical Examination form completed and signed.

Please make your appointment in advance. We only perform 2 sports physical
exams per day, so get it done before August!! Otherwise, there might be a
long waiting list......
